• Donation Form (to be completed for all donations that are not collected directly online for tax receipting purposes).
• Summary of Donations Collected (summary of cash and cheque donations collected by an individual participant or by a team as a whole).
• Team Member List (form to be filled out if members have not registered online; we will then add them to your team for you.)
