Application form of the Psychosocial Support Fund

Personal information

If you already have an online donation or application account with Fondation des artistes, use the log-in (“CONNEXION”) button at the top of the page. This will save you from having to re-enter your details.


This Psychosocial support fund, funded by the Conseil des arts et des lettres du Québec and managed by the Artists' Foundation, aims to provide free consultations to professional artists and cultural workers in the cultural sector who do not have access to psychosocial services, whether or not they are members of an association or group.

Only one application per person will be accepted.

To benefit from this psychosocial support program, you must
- Be a professional artist or cultural worker from Quebec;
- Have worked in the artistic field in the past two years and earned most of your income from your artistic profession;
- Not have received support from the Psychosocial Support Fund in 2024 or from the Equilibrium Program;
- Be 18 years or older at the time of your application.

Before starting your application, make sure you have the following document:
- Your current resume (curriculum vitae) including the years of your contracts.
- Proof of permanent residence: a photo of your health insurance card or valid driver's license.
