Application form of the Psychosocial Support Fund

If you already have an online donation or application account with Fondation des artistes, use the log-in (“CONNEXION”) button at the top of the page. This will save you from having to re-enter your details.

This Psychosocial support fund, funded by the Conseil des arts et des lettres du Québec and managed by the Artists' Foundation, aims to provide free consultations to professional artists and cultural workers in the cultural sector who do not have access to psychosocial services, whether or not they are members of an association or group.

Only one application per person will be accepted. If you have already benefited from the Psychosocial Support Fund before, unfortunately you cannot apply again.

To benefit from this psychosocial support program, you must
- Be a resident of Quebec for tax purposes;
- Be 18 years and over on the date of your application;
- Be a professional artist or cultural worker (demonstrate at least 4 years of professional work).

Before starting your application, make sure you have the following document:
- Your current resume (curriculum vitae) including the years of your contracts.
